If You Are Able to Answer "No" to All the Medical Questions

Then you Do Not need a doctor's note to Scuba Dive

Please text us as (808) 482-0683 to let us know that you reviewed the medical questionnaire and all questions are "No" Please include your first and last name and first and last name of all divers in your group (if there are more divers than just you) letting us know that everyone can answer "No" to the medical questionnaire.

Before confirming your reservation, you must have a Physician’s approval. Please download the “Participant Questionnaire and the Physician’s Evaluation Form” AND have your Physician sign for your approval to Scuba dive.
